What the Research Actually Shows
Heart Health — Adjunctive Use
The landmark Q-SYMBIO trial (Mortensen et al., 2014) tested CoQ10 300 mg/day versus placebo added to standard heart-failure therapy in 420 patients over 2 years. The CoQ10 group experienced fewer major adverse cardiovascular events, lower cardiovascular and all-cause mortality, and fewer heart-failure hospitalisations. A 2024 meta-analysis of 33 RCTs (Xu et al.) confirmed reductions in all-cause mortality and heart-failure hospitalisations, plus improvements in ejection fraction and walk distance when CoQ10 was used as adjuvant therapy. Importantly, these benefits were observed on top of conventional medications — CoQ10 is not a replacement for prescribed treatment.
Statin-Associated Muscle Symptoms (SAMS)
Statins can lower CoQ10 levels. A 2025 meta-analysis (Kovacic et al.) of seven RCTs found a significant overall reduction in muscle pain intensity with CoQ10 supplementation. Results across individual trials are mixed, and more research is still needed, but the data suggest CoQ10 may help some people maintain comfort and adherence to statin therapy.
Healthy Ageing & Energy
CoQ10 production declines with age. Its central role in mitochondrial ATP production and antioxidant defence makes it a logical candidate for supporting cellular energy in older adults. Direct long-term trials on lifespan or broad healthspan endpoints in healthy people remain limited. The most robust outcome data still come from cardiovascular and statin-related contexts.
